Headquarters
Headquarters will be located at the BE United FC Field-house in Perry, UT. (2825 S Hwy 89 #5, Perry, UT 84302)
Team packages will be available for pick up starting Wednesday at 12pm-2pm, Thursday June 20th 12pm-2pm and Friday June 21st 8am-10am.
Daily headquarter hours will be posted on our website.
On-line Check-In is mandatory and due no later than
June 16th
. Documents need to be emailed to

REQUIRED DOCUMENTS TO BE EMAILED
Rosters
:
Utah teams or teams in the UYSA system:
Official Sports Affinity UYSA Team Roster (coach and registrar signature on the bottom of form not required)
AND
Official UYSA/Affinity generated Tournament/Travel Roster with photos. This roster MUST list every player, with jersey number and coach associated with the team, including guest players.
Out of state teams
An Official Approved/Certified Roster from your State/National Association that list every player with a jersey number
AND
A Current Photo Player Card for every player (including guests) and every coach. Please scan multiple cards to an 8.5"x11" sheet of paper and upload them. Coaches cards can be scanned separately.
Guest Player Forms
(if applicable)
If you have guest players, you must have a Specialty Player Action Form (Guest/Loaned Player Form) signed by the loaning coach or club. Club passed players from within your club still need this form.
It is recommended for Team Managers to have copies of signed medical releases on hand in case of an emergency. These forms DO NOT need to be uploaded.
**Multi-Rostering of Players is not allowed per UYSA and US Youth Soccer sanctioned tournament rules . If a player is found to have played on more than one team, all games the player participated in will be forfeited**
Any requested changes to an approved roster must be emailed to goldenspikeclassic@gmail.com no later than June 16th.
